下のカレンダーよりご予約日時を１つだけお選びください。
＊２箇所以上に入力された場合は、全てが消去されます。お気をつけください。
予約後のご案内は24時間以内にEmailで送信されます。

インフルエンザ予防接種のご予約
【接種対象者】生後6ヶ月以上64歳以下の方 (65歳以上の方はファーマシーで接種してください。)
ご予約日の前に：
- 保険証のコピー（表と裏両方）とポリシーホルダーのお名前と誕生日をメールで[email protected]までお送りください。
- インフルエンザ予防接種の質問表/同意書をダウンロードしてご記入ください。
ご予約日にお持ちいただくもの：
- 記入されたインフルエンザ予防接種の質問表/同意書
- 保険証（海外旅行保険は対象外です。保険がない方は自己負担となります。）
For Flu Vaccine Appointment

Prepare for your appointment：
- Bring your completed Flu Vaccine Questionnaire & Consent Form (the form can be downloaded below)
- Bring your insurance card
- If you are a new patient, please send a copy of your insurance card (front and back with name of policy holder/date of birth) to [email protected]
- Wear short-sleeve shirt
- Plan to remain in the area for 15 minutes after your vaccination for observation
*We ask that you please refrain from calling our office to ask questions regarding the COVID-19 vaccine or Flu vaccine as it places volume overload on our telephone system and limits ability of other patient calls that require urgent attention.
